Dans le domaine de la santé, il est de plus en plus fréquent de voir le gouvernement remettre à plus tard des décisions, sur la base d’un manque de données probantes. Il arrive cependant des surprises, comme l’annonce du gouvernement fédéral en mars 2007 de l’octroi de 300 millions de dollars pour la mise sur pied d’un programme de vaccination contre le virus du papillome humain: décision médicale, choix politique et commercial.

Updated information on HPV vaccination safety

In June 2008, the media reported that the Food and Drug Administration had received over 8000 reports of serious adverse events following HPV vaccination with Gardasil. This, of course, raised consumers’ and parents’ concerns regarding the safety of this vaccination.

However, according to information provided by the Centers for Disease Control and Prevention (CDC), it seems that no links were found between these adverse events and the vaccination itself.

Bien que le Canada ne soit pas confronté à une épidémie de cancer du col de l’utérus comme en Afrique ou en Amérique latine, des coûts importants sont reliés au virus du papillome humain (VPH). Pour cette raison et plusieurs autres, Helen Trottier, épidémiologiste spécialisée en maladies infectieuses et chercheure au CHU Sainte-Justine, soutient le programme de vaccination contre ce virus proposé par le gouvernement canadien. Dans cette entrevue, madame Trottier fait le point sur les bénéfices, les risques et les limites de la vaccination contre le VPH.

HPV Vaccination: An epidemiologist’s viewpoint

While not faced with an epidemic of cervical cancer like that seen in Africa or Latin America, Canada must still contend with some significant costs related to Human Papillomavirus (HPV) infections. This is one of the many reasons why Dr. Helen Trottier, an infectious diseases epidemiologist and researcher at the Sainte-Justine University Hospital Center in Montreal, supports the HPV vaccination program proposed by the Canadian government. In this interview, Dr. Trottier provides a brief overview of the benefits, risks and limitations of HPV vaccination.

Les maladies et les cancers reliés aux VPH : la vaccination, une solution d’avenir

Chaque année, 465 Québécois seront diagnostiqués d’un cancer de l’anus ou des organes génitaux. Malgré les efforts investis depuis 50 ans dans le dépistage du cancer du col de l’utérus, 325 Québécoises développeront un cancer du col utérin et, malgré des traitements adéquats, 80 d’entre elles en mourront.

Une campagne de vaccination contre le VPH ne pourra se faire sans mobiliser d’importantes ressources professionnelles et financières. Compte tenu du fardeau de la maladie qu’elle prétend enrayer et de la prévalence d’autres problèmes de santé sexuelle criants dans notre société, l’investissement en vaut-il la chandelle? La Fédération du Québec pour le planning des naissances remet en perspective les enjeux de la vaccination contre le VPH.

Do you approve of spending $300 million on HPV vaccination?

This is the question that the journal Canadian Family Physician put to some experts in its February 2008 edition.

Dr. Marc Steben, family physician at the INSPQ, believes the government’s investment in HPV vaccination is fully justified given that HPV represents an enormous burden for Canada, current methods for preventing HPV infections have serious limitations, and the HPV vaccine is safe and effective. On the other hand, Abby Lippman, Professor in the Department of Epidemiology at McGill University, Madeline Boscoe, Advocacy Coordinator for the Women’s Health Clinic in Winnipeg, and Dr. Carol Scurfield, Medical Director of the Women’s Health Clinic in Winnipeg call for a more cautious approach. They advise not rushing into such a program, noting that the funds to create the needed public health infrastructure have not yet been provided. They also explore the many relevant questions that remain unanswered.

Human Papillomavirus Vaccines: A Canadian Update

In July 2006, a vaccine for the prevention of infections caused by the human papillomavirus (HPV) was approved for sale in Canada.

A second vaccine is currently undergoing Health Canada review. In its December 2007 issue, The Canadian Agency for Drugs and Technologies in Health (CADTH) summarizes the most recent evidence on the efficacy and safety of these vaccines. It also presents the results of some cost effectiveness analyses and discusses a number of unresolved questions about HPV vaccinations: ideal age for immunization, duration of effect, immunization of women already infected, vaccination of males, implication for Papanicolaou (PAP) smear programs, barriers to uptake, need for monitoring and registries, cost-effectiveness, and programs to ensure access for special populations.
